What does Practice Direction 32 require for witness statements?

PD 32 requires the statement to be headed with the case details, contain the witness's full name and address, be in the witness's own language, include a statement of truth, and be signed and dated. Exhibits must be separately identified and verified. The statement must also indicate how it was prepared (e.g., from a telephone conversation or written notes).
